Jeremiah Cook, "B" Co., 109th Regt. U.S.C. Troops was. born in the state of Kentucky, was enlisted the twelfth day of June, 1864, at Bowling Green KY, by Captain Hobson, and was mustered in the twelfth day of June, 1864, at Bowling Green Ky, by Captain Hobson, for three years.
At the time of enlistment he was twenty-five years old, five feet ten inches in height. He was discharged [[strikethrough]] (or died) [[/strikethrough]] third day of June, 1865, at Norfolk Va; Captain Ed B Keplinger was commanding officer, and Robert E. Coving First Sergeant at time of soldier's discharge. The following are the names of one or more of his company who enlisted at or about same time and place: Private Tobias Canon enlisted tenth day of June, 1864, and Private Charles Hix enlisted twelfth day of June, 1864.
