Frank Lewis, Private Co., "J" 4th Reg't U.S.C. A (Ky) was born in Hickman Ky Moscow, Kentucky, was enlisted the 8th day of November, 1863, at Columbus, Ky., by Lt. Brewer, and was mustered in the 22nd day of December, 1863, at Columbus, Ky, by Lt. Fetterman, for 3 years.

At the time of enlistment he was 22 years old, and 5 feet 6 inches high, Black complexion, Black eyes, Black hair, and by occupation a Farmer. He was discharged [[strikethrough]] (or died) [[/strikethrough]] 25th day of February, 1866, at Pine Bluff, Ark. 

Capt. Daniel Matoon was commanding officer, and Isaac Smith first sergeant at time of soldier's discharge. [[strikethrough]] The following are the names of one or more of his company who enlisted at or about the same time and place: enlisted day of 186 , and enlisted day of , 186 .
