NARA 0858 (Form 24.) York Phason, Sergt. Co., "H" 4th Reg't U.S.C A. (Ky.) was born in Samson Co. N.C., was enlisted the 15th day of October, 1863, at Columbus, Ky., by [[strikethrough]] Lt. Johnson, [[/strikethrough]] and was mustered in the 23rd day of October, 1863, at Columbus, Ky., by Lt. Fetterman, for 3 years. At the time of enlistment he was 23 years old, and 5 feet 5½ inches high, Black complexion, Black eyes, Black hair, and by occupation a Blacksmith. He was discharged [[strikethrough]] (or died) [[/strikethrough]] 25th day of February 1866, at Little Rock, Ark. Capt. E Kenneys was commanding officer, and Alexander Shaw first sergeant at time of the soldier's discharge. [[strikethrough]] The following are the names of one or more of his company who enlisted at or about the same time and place: _____________enlisted ___day of _______ 186 , and ______________ enlisted ___ day of ________ , 186 .
