NARA 1011 (FORM 24.) George Yantis, Private Co., "H." 116th Reg't U.S.C. Troops was born in Garrard Co Kentucky, was enlisted the 6th day of July, 1864, at Camp Nelson Ky, by J. C. Randolph, and was mustered in the 12th day of July, 1864, at Camp Nelson Ky, by Captain Kinney, for 3 years. At the time of enlistment he was 23 years old, and 5 feet 8 inches high, Black complexion, Black eyes, Black hair, and by occupation a Farmer. He was discharged [[strikethrough]](or died) [[/strikethrough]] 17th day of January, 1867, at New Orleans La 1st Lieut't Phil. Weitzel was commanding officer, and Meade Hightower first sergeant at time of soldier's discharge.
