From a plant in cultivation. Wm. C. Werner's herberium was in Ohio. Presumably plant was being cultivated there. Given that an herbarium is a collection of plants from, potentially, many regions, it's definitely an assumption that this specimen was collected in Ohio. However, since C. Werner was on the faculty of Ohio State University and collected extensively throughout Ohio (see, for example: https://goo.gl/b9JzH7), it seems to be a fairly safe assumption. -@mnathan
